The ability to perform initial assessments, mental status observations, professional consultation, life care planning, and give expert testimony are required. Typing documentation of interactions with clients and other professionals is required, as well as ability to learn electronic medical records software. Candidate will have his/her own case load of clients to care for, and participate in a monthly clinical rounds and staff meeting for continuity of care purposes. Candidate will be provided with an appropriate office space including a personal desk, computer, professional email account, cell phone for business purposes. Position also includes 401K, as well as mileage reimbursement. Pay is competitive and will commensurate with experience.
CRCI is seeking a part-time or full-time case manager to provide outpatient case management for clients who have sustained work or auto related traumatic injuries.
This is a brief position description of medical case managers. CRCI case managers assist injured survivors and their families to get the best possible service, treatment, care planning and implementation of care plans concerning their case. All medical case managers must obtain the best treatment options, as well as coordinating cost effective services to the advantage and well-being of the survivor. Medical case managers collaborate with professionals and meet with them and the clients in the field to coordinate and tailor services specific to the client’s needs. Planning is important to management and is accomplished by establishing a set of meaningful goals, objectives and action steps which is the basics of case management. Organization is as equally important to management, by structuring of material, resources, time and people as it relates to the care of the client. Case management involves strong leadership skills by taking on the responsibility of some decision-making and problem solving expertise by communication, motivation, advocacy, etc. Case manager assists client and team in making appropriate goals for medical treatment as well as addressing aspects of case such as family dynamics, community re-entry, vocational assistance, etc. All treatment is driven by the physicians and prescriptions for care.
